Improved survival rates from cancer means there is a growing number of people living with the long term consequences of the disease and its treatments. Recent years has seen a growing body of evidence to support interventions that minimise the risk of occurrence of adverse effects and reduce the impact of disease and treatment related effects on cancer survivor’s health and wellbeing.  These interventions focus on health promotion and enabling self-management for cancer survivors， as well as ensuring effective transitions across health care settings.  However， there are a range of sociocultural， professional and health system related factors that influence the extent to which such interventions are implemented in current practice.    Health professionals need to understand the changing needs of cancer survivors and contribute to redesigning health care systems that are more responsive to the needs of the growing population of cancer survivors.
